General Liability Certificate Request
We will be happy to send a certificate of liability insurance to you or to whomever you direct. If you are required to name the certificate holder as an additional insured we need to know if the requirement is verbal or in a written agreement or contract. Please be advised the insurance company may charge a small additional premium for specifically naming an additional insured and they will bill you. If the certificate holder is requesting the standard Acord form to be altered or special wording inserted, you need to call us to discuss the specifics.
